Q: Do you have a cancellation policy?
A: Yes, we require 24-hours notice for cancellations. All cancellations under 24 hours and no showed appointments will result in a fee for the appointment. Please remember that we set aside the time for you, we expect you to do the same. We understand that life happens, therefore certain circumstances will be taken into consideration.

Q: Where do I find my pre-screening COVID questionnaire?
A: It will be sent to you in your reminder email, 24 hrs prior to the scheduled appointment time, and it must be filled out prior to every appointment moving forward.
